A Lancashire coroner has criticised several NHS trusts after a young man's psychotic illness went undiagnosed for years before he killed his granny.

When Ms Middlebrook, who still works at the mental health liaison team, finally carried out an assessment on Conor at around 5am on July 3 she claimed it had lasted around 45 minutes. However, the inquest heard that police tracking systems showed Conor was back at home by 5.39am much to the shock and disappointment of his granny and aunt.

After Conor had been sent home from hospital his aunt visited Whitegate Manor in Blackpool where he was due to have a mental health assessment nine days later. She took a 10-page letter in which Marlene had written that she was terrified and warned of how Conor could "erupt like a volcano". "Mrs Whitlow told her to put the key in the door in case she needed to get out quickly but she failed to do so. She left at 5.10pm and returned at 5.50pm to find Marlene dead on the floor of the kitchen. Conor entered and when Mrs Whitlow asked what he had done he replied 'that's what you get when you mess with Lucifer'. He then started to chase her, skipping and laughing, and struck her on two occasions.

The coroner recognised that the NHS trusts involved in Conor's care had taken significant steps to improve ways of working as a direct result of Marlene's death. Several trusts were legally represented during the course of the inquest. Victoria Beel, Principal Lawyer at Slater and Gordon, who represented the family, said: “The Coroner found that there was an arguable systemic dysfunction in communication between the mental health providers involved in Conor’s care.
