A Review published in BMCGastroEnterol summarizes the evidence base that could help in discriminating gastrointestinal tuberculosis from Crohn’s disease. It highlights the ongoing research that might improve this discriminative ability.
The bowel can be evaluated by plain radiographs, ultrasound , barium studies, computed tomography , magnetic resonance imaging , and positron emission tomography -CT. The role of abdominal radiographs is limited except in emergencies like acute intestinal obstruction or suspected gastrointestinal perforation. A chest radiograph may be useful as approximately one-fourth of GITB patients may have evidence of pulmonary TB [].
Fleischner’s sign due to patulous and gaping IC valve associated with narrowing of the terminal ileum, and Steirlin sign, which is rapid emptying of barium from the caecum to ascending colon due to irritable caecal mucosa may be seen in GITB. The other features are purse string stenosis opposite to the IC valve associated with dilated terminal ileum [].

Concomitant aortic regurgitation predicts better left ventricular reverse remodeling after transcatheter aortic valve replacement - BMC Cardiovascular DisordersBackground We aim to determine predictors of inadequate left ventricular mass index (LVMi) regression at mid-term after transcatheter aortic valve replacement (TAVR), including the potential role of epicardial adipose tissue (EAT). Methods We retrospectively reviewed patients with both echocardiographic assessments and multi-slice computed tomography (MSCT) obtained one year after TAVR. The change of LVMi, the volume and the average CT attenuation of EAT from baseline to one-year follow-up was calculated. Patients were divided into two groups by the percentage change of LVMi at a cut-off of 15%. Results A total of 152 patients were included with a median age of 74 years (interquartile range [IQR] 69–78 years) and 56.6% being male. LVMi decreased (P mild (P = 0.001) and the change in the average CT attenuation of EAT (P = 0.026) were different between the decrease of LVMi ≥ 15% and mild at baseline was the only statistically significant predictor of a decrease of LVMi | 15% at one year (OR 0.33, 95% CI: 0.13 to 0.84, P = 0.021) in multivariate regression. Conclusions Concomitant more-than-mild AR might predict better left ventricular reverse remodeling regression after TAVR.

Association between breastfeeding and periodontitis in Korean women using Korea National Health and Nutrition Examination Survey (KNHANES): a cross-sectional study - BMC Oral HealthObjectives The effect of breastfeeding on periodontal disease in women remains unclear. This cross-sectional study used data from the Korean National Health and Nutrition Examination Survey to explore the association between breastfeeding and periodontitis in Korean women using data from the Korean National Health and Nutrition Examination Survey (KNHANES VII). Materials and methods Cross-sectional data was analyzed from the KNHANES 2016–2018. The study population included 5,587 parous women aged ≥ 30 years. The outcome variable was the presence or absence of periodontitis. The explanatory variable, period of breastfeeding, was defined as “none”, “1–11 months”, and “more than 12 months”. Confounder variables (socio-educational, personal healthcare practice, and systemic medical characteristics) were adjusted for in the logistic regression analysis. Results Approximately 60% of the participants breastfed for ≥ 12 months. In all statistical models, the prevalence of periodontitis was approximately 60% greater in women that did not breastfeed compared to women that had breastfed for 12 months or longer. When adjusted for age, statistical significance was only present in the 50–59 years age group (adjusted odds ratio [aOR], 1.678; 95% confidence interval [CIs], 1.046–2.691). Conclusion Our study shows that women that breastfed for a relatively long duration had a lower risk of periodontitis. Therefore, breastfeeding may be beneficial for women’s periodontal health. These results are expected to be helpful in oral health education for pregnant women.

Parents’ experiences of life after medicalised conception: a thematic meta-synthesis of the qualitative literature - BMC Pregnancy and ChildbirthBackground Medicalised Conception (MAC) assists many couples to achieve pregnancy worldwide. As the impact of MAC has been linked to increased pregnancy-specific anxiety and parenting difficulties, this review aimed to explore parental experiences of pregnancy and early parenting following MAC, identifying parents’ psychological, social and health needs. Method Five databases were searched systematically from inception to March 2023. Identified articles were screened for eligibility against the inclusion criteria and the results were analysed using thematic synthesis. The Critical Appraisal Skills checklist was employed to appraise methodological quality. Results Twenty qualitative studies, drawing on a total of 19 participant samples, were included in this review, most with samples with history of subfertility. The findings were synthesised into three main themes (consisting of seven subthemes): 1) The vulnerable parent: fear, doubt, uncertainty, 2) the stark realisation of the parental dream, 3) psychosocial needs and support. Parents lacked a sense of safety during pregnancy and reported acting protectively both antenatally and postnatally. Furthermore, their identity transition was complex and non-linear, influenced by sociocultural context. Conclusions Considerable unmet psychosocial needs were identified including the potential for anxiety in pregnancy, the possibility of feeling excluded and marginalised, and a reluctance to share distress and experiences with healthcare professionals. These findings suggest a need for consistent, holistic care, integrating psychological services.
