Stewart Glancy’s care did not always “meet the standards expected”, a review has found
The findings have angered Stewart’s family, who are still awaiting a fatal accident inquiry more than three years after the dad of three, 38, was found dead in his cell at HMP Addiewell in June 2020. The cause of death was bleeding from a gastric ulcer.
“I can’t accept that he didn’t ask to see a doctor until the day he died. Why would anyone wait till then? I’m convinced they could have saved my boy and that somebody is covering things up. On June 2 Stewart fainted but his blood pressure was not recorded as the machine was not working. However, it was known he had an increased heart rate, which would have raised his National Early Warning Score .
On the day he died Stewart complained of back pain and feeling faint. He was pale and struggling to breathe. He was to be checked by guards every 30 minutes but was found dead in his cell that evening. The report conceded there were “omissions in the patient’s care that sadly contributed to his death”.
