Vulnerable man choked to death on raw chicken but carer still insists he wasn't at risk
A service which provides supported living for people with mental health difficulties faced heavy criticism from a coroner after a man died by choking on raw chicken.
In a damning report to prevent further deaths, Mr Wilson made clear his criticisms of the company and the service manager and ordered improvements to be made. Noting his concerns that the manager was unlikely to make changes or implement them at speed if he did, he has used his powers to legally compel the service to respond.
Mr Wilson observed: "The risk that Steven could choke on his food had not been fully appreciated, but from the available evidence it cannot be established that a fuller appreciation of the risk would have averted Steven’s death. The conclusion of the coroner was that Stephen died an accidental death."
