A number of incidents took place across three different Trusts

A nurse 'could have put a patient at risk of death' after she failed to administer critical medicine while they were suffering from an epileptic seizure, a standards committee has heard.

Mrs Cornthwaite took full responsibility for her errors, the committee heard. She completed a reflective statement indicating steps she would take to ensure it would not happen again. There was no patient harm, and Mrs Cornthwaite correctly escalated the incident to the ward sister and completed the appropriate report. Notwithstanding this, the committee heard she should not have used a drug from another patient's stock or given a drug without the drug chart being present. Mrs Cornthwaite wrote a reflective statement following the incident.

The prescription chart had 'CRITICALMEDICINE' next to the name of the drug on the prescription chart. Another nurse found the medication in the ward's medicine trolley and administered it immediately.
