A prisoner died from an overdose after jail staff failed to notice he had been stockpiling the antipsychotic medication in his cell, an inquiry heard. Robert Allen, who passed away in May 2019, was in the segregation and reintegration unit at HMP Shotts in Lanarkshire, due to two incidents involving serious violence.

He was assessed as posing a "significant risk" of further serious violence due to refusing to take the drugs. A prison medic, Dr Morris, decided that the limit of care that could safely be provided in a prison environment had been reached by March and that he should be moved to the state hospital. Prior to his death, Mr Allen told staff he wasn't complying with antipsychotic medication on more than one occasion. The inmate then died on May 20, 2019, as a result of Methadone and Quetiapine intoxication.
